The Staff of Polhemus Cremation Services is committed to providing professional and affordable cremation services and direct burials.
Federal Trade Commission Disclosure: The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any cremation funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the cremation funeral foods and services you selected.

BASIC SERVICES OF THE FUNERAL DIRECTOR, STAFF AND OVERHEAD
Federal Trade Commission Disclosure: This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct creations, immediate burials and forwarding or receiving remains.)
New Jersey State Disclosure: This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.)
This fee includes the following:
-Personnel available 24 hours a day, 365 days a year to respond to initial call
-Arrangement conference
-Coordinating service plans with crematory, cemetery, and/or other parties involved in the final disposition of the deceased
-Clerical assistance in the completion of various forms and permits associated with a funeral.
-Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administrative expenses and governmental compliance.............................................$995.
PREPARATION OF THE BODY
Federal Trade Commission Disclosure: Except in certain special cases, embalming is not required by law. Embalming is not required for direct cremations or immediate burials.
New Jersey State Disclosure: Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. If you choose to have an embalming performed, the decedent may be embalmed at a location other than this registered mortuary.

FORWARDING REMAINS TO ANOTHER FUNERAL HOME
This charge includes use of preparation room, basic services of staff, embalming, transfer of the remains to the funeral establishment, transportation to or from local airport or other place of shipment with a 20 mile radius and obtaining necessary authorizations. This charge does not include visitation or ceremonies. This charge applies to shipment with the continental U.S. only. $2,500.
RECEIVING REMAINS FROM ANOTHER FUNERAL HOME
This charge includes basic services of staff, transportation of remains to cemetery or crematory, transportation from local airport or other place of shipment with a 20-mile radius and obtaining necessary authorizations. This charge does not include visitation or ceremonies.

DIRECT CREMATION $1,295. to $4,000.
Federal Trade Commission Disclosure: If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of wood, containers made of fiberboard and containers made of other composite materials.
New Jersey State Disclosure: If you want to arrange a direct cremation, you can use an unfinished wood box or an alternative container. Alternative containers can be made of materials like heavy cardboard or composition materials (with or without an outside covering) or pouches of canvas. The containers we provide are alternative containers and other cremation containers, including containers made of wood, containers made of fiberboard and containers made of other composite materials. See descriptions below.
Our charges for direct cremation (without ceremony) include Basic Professional Service Fee, Local Transfer of Remains to the Funeral/Cremation Establishment, Transfer of Remains to the Crematory. (Does not include crematory fee, permit or transcripts)
